For W-2 Employees:

  • Most recent 2 months of pay stubs (must be consecutive)
  • Most recent 2 years of W-2 forms
  • Most recent 2 years of federal tax returns (all pages, all schedules)
  • If recently employed: offer letter or employment verification

For Self-Employed:

  • Most recent 2 years of personal tax returns (all pages, all schedules)
  • Most recent 2 years of business tax returns (1065, 1120, 1120S)
  • Year-to-date profit & loss statement
  • Business bank statements (3-6 months)
